Yes.
2. When was the last time you reloaded the plugin?
Just now.
3. Is there anything in gw/Guildwork.log ?
It's currently empty because I just reloaded the plugin, but I checked it prior to that and it only had the 404 on ffxidb.com/kills.

I am not behind a firewall, nor am I running AV software. If I run the shouts command, I get a dump of the most recent shouts.

You don't have an FFXI character added to Guildwork. Adding one and reloading the plugin will fix your issue.
